Your enrollment includes the creation of your personalized digital emergency medical card, bringing essential medical information, hospital preferences, insurance details and emergency contacts together in one centralized place.

$29.95 monthly membership fee

Your monthly membership maintains access to CostaCare's hospital transportation reimbursement benefit. Eligible transportation expenses are reimbursed according to membership tenure, with benefits increasing over time to a maximum of $500, subject to membership terms and conditions.
